Inter Med Associates accepts most insurances . Please verify with your insurance carrier to insure your physician is listed as a in network provider. It is the responsibility of the patient to update their primary care information with both their insurance carrier and provider’s office. Failure to do this may result in your medical claims being processed out of net-work or denied.

  • Co-pays are due at the time of appointment.

  • Forms of payment accepted: Cash, Visa, MasterCard, Discover, American Express and CareCredit. Personal checks are no longer accepted.

  • If you are more than 10minutes late for your scheduled appointment, your appointment may be cancelled.

  • Missed Visit Charge: A fee of $25.00 will be charged to your account for any missed appointment. Please notify your doctor’s office at least 24 hours in advanced to cancel or reschedule your appointment to avoid this surcharge.
 
Please bring to your appointment:
 
  • Current insurance card: Please present your current insurance card at time of check in.

  • Medication List: Please bring current list of your medications and any new medications that have been added since your last visit. Please complete and print Medication Form at www.webstermd.com

  • Medical Records: If you are new the practice, please have your medical records transferred to your new pri-mary care physician prior to your appointment if possible, otherwise bring to your appointment.

  • Demographic Change Form: Complete and print if you have had any changes to report. www.webstermd.com

  • Consent Forms: Complete and print if you are changing or adding new consents. www.webstermd.com
